HEALTH DOCUMENTS
Why is it absolutely necessary to have a HEALTH CARE SURROGATE form? As we have seen in the news and media lately, unless you are able to let a doctor, hospital or health care provider know what your wishes for medical treatment are, the decision may be made by others, not even by your own family!!!

In the State of Florida, decisions that are made about a person's health, when that person is not capable of making a decision, are not automatically made by family members or loved ones. Because of privacy laws, the wishes of the family may not be listened to and it may be necessary to have a guardian or other party appointed to make those decisions for you.

IF YOU ARE NOT CAPABLE OF MAKING MEDICAL DECISIONS FOR YOURSELF BECAUSE OF AN ILLNESS, LACK OF CAPACITY OR DISABILITY, YOU MUST HAVE A HEALTH CARE SURROGATE DOCUMENT ALREADY SIGNED AND PREPARED SO THAT YOUR SURROGATE CAN MAKE THOSE DECISIONS FOR YOU.

The Health Care Surrogate appoints someone you choose, who you know and trust, to make all medical decisions for you in the event you cannot make the decisions yourself. This applies whether your medical condition is temporary or permanent. Of course, if your incapacity is temporary, once you recover you are then able to make the decisions for yourself again.

A Health Care Surrogate form grants the authority to your chosen Surrogate to manage your medical care, make medical decisions for you and to advocate your wishes and desires when you are not able to. They speak for you when you are not able to!! They are able to decide if a second opinion should be had, if the suggested treatment for you is reasonable and they can consult with your family and loved ones regarding their wishes as well.
